Wisdom Teeth and Their Removal
The last teeth to erupt in our mouth are the wisdom teeth. Since they usually erupt in the twenties, they are also called by this name or wisdom teeth. In some people, no wisdom tooth germs have been formed at birth. Therefore, wisdom teeth never erupt in these people. However, sometimes these teeth cause eruption problems even though they are formed. It may be necessary to extract wisdom teeth due to lack of space in the jaw or for various reasons.
What are the situations that require the extraction of wisdom teeth? The last teeth to erupt in our mouth are the third molars. It usually starts between the ages of 17 and 25. If they erupt in the correct position and do not damage the surrounding tissues, there is no harm in keeping this tooth in place. A decision may be taken to extract a tooth that is fused to the jawbone and is in an abnormal position, taking into account the damage it will cause in the future. In cases of limited space in the dental arch, the eruption of the tooth may get stuck in the gum-bone and other adjacent tooth obstacles.
IN WHICH CASES SHOULD TWENTY-YEAR-OLD TEETH BE EXTRACTED?
Rotten
Saliva, bacteria and food particles accumulate in the socket created by the newly emerging tooth, threatening both the wisdom tooth and the molar tooth next to it. It is quite difficult to recognize and treat this type of caries. Severe conditions may occur, causing pain, infection, and resulting in abscess.
Gum Disease (Pericoronitis)
A partially erupted wisdom tooth creates a focus of infection in the gum where bacteria and food residues are stored. This situation causes bad breath, pain, edema and trismus (inability to fully open the mouth). The infection can spread to the cheek and neck via the lymph. This infection-prone ground around the wisdom tooth is a candidate to become infected easily every time.
Pressure Pain
During eruption, pressure may occur on adjacent teeth due to lack of space and pain may be felt due to compression. In some cases, this pressure leads to edema and swelling.
Reasons Related to Prosthesis
In a mouth where prosthetic planning is being done, wisdom teeth must be taken into account. Because, after the wisdom teeth are extracted, it will be necessary to make a new prosthesis according to the changing mouth structure.
Cyst Formation
Cases of cystic lesions caused by an impacted tooth have been observed. Cystic lesions cause bone destruction, spontaneous fractures in the jawbone, and displacement or damage of the surrounding teeth. To prevent bone destruction, the tooth must be extracted and the cyst must be cleaned. Rarely, if this cyst spreads to very large areas, it can turn into a tumor and cause spontaneous fractures to the jawbone.
Orthodontic Reasons
If there is no room in a completed dental arch, the erupting wisdom teeth must be extracted if they will disrupt the arch form.
It is an operation performed in cases where root canal treatment cannot be performed to eliminate infections that develop at the root tips of the teeth and to clean the inflamed root tip and the surrounding tissues where the inflammation has spread.
Abscess and Cyst Operations (Apical Resection)

- Inability to complete root canal treatment due to structural or deformity of the tooth root,
- Inability to perform root canal treatment due to the presence of a non-removable restoration on the tooth,
- If the tool is broken during root canal treatment and the broken tool must be removed, in order to remove the tool,
- In cases where the patient’s pain continues despite the root canal treatment,
- In cases where cysts occur on the tooth root,
- In cases where the tip 1/3 of the tooth root within the bone is broken.
